Apartment Treatment Program, Community Residence (Scattered Sites)
This is a NYS Office of Mental Health licensed housing program designed for single adults diagnosed with a mental illness who will benefit from a shared living arrangement as they work on the skills that will enable them to successfully transition to more independent supportive housing. Residents have their own bedrooms in a scattered site, three-bedroom apartment. Individuals may remain in the Community Residence for as long as two years or while they continue to require services at this level.
Emphasis is placed on assisting residents to move to their own community apartments or other appropriate housing setting. Apartments in this program are for residents who do not require 24-hour on-site supervision and have reached a level of functioning where need for staff assistance or training can be met through staff visits to the apartment. Residents in the Apartment Treatment Program have access to staff 24 hours a day for crisis intervention.
Supportive Housing (Scattered Sites)
This program is designed as permanent housing for individuals, some with families, who have a mental health diagnosis who can benefit from supportive services. Support services are [provided either in the individual’s home or at the Supportive Housing Office. Residents have access to the program’s 24-hour on call system. The services include an assigned case manager in a collaborative, goal oriented manner allowing for client choice and respecting individuality and independence.

These programs provide permanent housing with on-site case management services during normal business hours to the residents who reside in studio apartments. Twenty-four-hour, on site security is provided. Residents have access to the program’s 24-hour on call system. The services include an assigned case manager in a collaborative, goal oriented manner allowing for client choice and respecting individuality and independence.

Other personal information that we collect is important to run our programs, to improve services for homeless individuals, and to better understand the needs of homeless individuals.
